Note: Researchers at Curtin University are doing an independent evaluation of the pre-planning tool with the aim of improving it for future use. They would like to talk to people who have completed the tool, either as the principal or on behalf of the principal. Curtin University Human Research Ethics Committee (HREC) has approved this study (HRE2019-0223).

This tool has been developed by Provider Choice, an organisation that is an NDIS registered plan manager who also teaches people how to self manage.
